Contract April 1.
We want to inform you of an important change that may impact your clients enrolled in a SCAN Medicare Advantage plan with P3 Health Partners. Effective April 1, 2025, P3 Health Partners will no longer be in-network with SCAN.
Patients will receive a letter notifying them of this change an encouraging them to select a new Medicare Advantage plan if they wish to continue receiving care from their trusted providers and specialists.
Your support is critical in ensuring a smooth transition for your clients. The Medicare Open Enrollment Period (OEP) ends March 31, 2025, so it’s essential they select a new plan before this deadline.
2025 Medicare Advantage Plans Contracted with P3 Health Partners Network:
✅ Aetna – (Prime Plus, Select, Value Plus, Dual Prime, Choice, Platinum, Eagle, Prime Plan)
✅ Alignment Health – (Platinum + Instacart, Smart, Heart & Diabetes, The One)
✅ Humana – (Gold Plus, Gold Diabetes & Heart, Choice, Honor Giveback, Giveback)
✅ WellCare – (Simple, Dual Access, Specialty Simple)
Some affiliate providers may be contracted with additional plans.
